What should I bring to my first appointment?

For a footcare appointment please bring the following information:

•     Completed Patient Information Form
•     List of current medications
•     Third party insurance information.  Please check coverage and eligibility for these services.

For an orthotic appointment please bring the following information:

•     Completed Patient Information Form
•     List of current medications
•     Third party insurance information.  Please check coverage and eligibility for these services.
•     Everyday shoes and/or shoes such as athletic shoes that you intend to use the orthotics in
•     Any existing/previous orthotics  you have in your possession
